Thank you for choosing to use our website to complete your
database form for the 2006 Commonwealth Games in Melbourne.
The information you submit will be used by the Commonwealth
Games Council for England (CGCE) for administrative purposes
only. No information will be passed onto other parties except
for those parts relating to the accreditation and entry requirements
of the Games Organising Committee.
Every potential team member of Team England has been asked
to complete this form, to enable for a smooth administration
process, should you ultimately be selected as part of the
team.

| In order for you to access the database
form, you should use the password supplied by your Team Manager.
If you haven't yet received this then please either contact
your Team Manager, or Caroline
Tyler. The password is case sensitive so
must therefore be typed in lower case with no space
in between the two words. |
You should also send 4 colour passport photographs
to the CGCE office addressed to:
Accreditation,
Commonwealth Games England, PO Box 36288, London SE19 2YY
The photographs need to conform to the following:
The size of the photograph must be 45mm x 35mm (passport
sized);
It must be a colour original on a light plain background;
Hats, hair bands or dark glasses may not be worn, unless for
medical or religious reasons;
Head and shoulders shot, showing full face;
The image must be a recent and a true likeness of you; and
You must write your full name and date of birth on the back
of each photograph you submit.

| Alternatively, the Organising Committee
will accept a digital photograph, as long as it complies with
the above and is in a jpeg file format - not exceeding 50kb,
with a resolution of 300 dpi. Digital photographs should be
sent to c.tyler@cgce.co.uk.
Please make sure you have named your photograph ie. use your
first name and surname before you email the photograph. |

Any photographs submitted that are not as described above
will be returned to the sender and new ones requested.
If we do not receive the correct photographs from
you then your place on the Team may be at risk, as the photographs
are required to obtain the accreditation pass of selected
team members for the Games.
You should complete the form as fully as possible
and ensure that all size measurements are
filled in. This will ensure that clothing supplied to selected
team members at the Games will fit properly.
